What actually determines the cost of IVF

Why there is no single price

Couples usually start by searching for a number, and the internet obliges with ranges wide enough to be useless. The reason those ranges are so wide is not that clinics are hiding something. It is that IVF is a set of decisions, and your evaluation determines which decisions apply.

A 28-year-old with good ovarian reserve and a clear pelvis follows a different protocol from a 39-year-old with endometriosis. Same clinic, same doctor, genuinely different treatment — and therefore genuinely different cost.

The line items that actually move the total

1. Medication protocol and dosage

Usually the biggest variable of all. Stimulation drugs are dosed against your ovarian response, which is assessed from age, AMH and antral follicle count. Two patients on the same nominal protocol can need very different quantities. This is also why a quote given before your baseline tests is not a real quote.

2. Whether ICSI is required

In conventional IVF, sperm and egg are combined and fertilisation is left to happen. In ICSI, a single sperm is injected directly into the egg — used where there is a male factor or previous fertilisation failure. It is an additional laboratory procedure with an additional cost.

3. Number of monitoring visits

A stimulation cycle needs repeated scans and blood tests across roughly two weeks to time the trigger and retrieval correctly. Cycles that respond unusually need more. For couples travelling in from Rajula or Savarkundla, these visits also carry a real travel cost that quotes never include — worth counting when you compare a local clinic against one in Rajkot or Ahmedabad.

4. Embryo freezing and storage

Surplus good-quality embryos can be frozen for later transfer. That adds a freezing charge and an ongoing storage fee — but a subsequent frozen embryo transfer is substantially less expensive than a fresh cycle from the start. It often lowers the cost of a second attempt considerably.

5. Preparatory surgery

Sometimes the obstacle is structural: endometriosis, a fibroid distorting the cavity, an ovarian cyst, a blocked fallopian tube. Treating it adds a procedure to the bill. Not treating it can mean paying for cycles that were never going to work.

6. Investigations before treatment

Hormone panels, semen analysis, imaging and infection screening for both partners. Modest against the cycle cost, and the thing that makes every later decision sound. Bring any recent reports you already have — repeating a valid test wastes your money.

7. How many cycles

The uncomfortable one. IVF success per cycle is not 100% at any age, at any clinic, anywhere in the world. Budgeting for a single cycle when your situation realistically suggests more is the most common financial mistake couples make. Ask your doctor directly what is realistic for your case, and plan against that answer rather than the best case.

Questions worth asking any fertility clinic

• What exactly is and is not included in the figure you have quoted me?
• Is medication included, or billed separately at actual cost?
• How many monitoring scans does this protocol typically need?
• What happens to the cost if the cycle is cancelled before retrieval?
• What does freezing cost, and what is the annual storage fee?
• Given my evaluation, what is a realistic number of cycles?
• Is there a non-IVF option — surgery, IUI, or waiting — worth trying first?

That last question matters most, and a clinic that answers it honestly is telling you something about how it practises.
